What Is Top Surgery?

Top surgery, also known as chest masculinization surgery, is a gender-affirming procedure that removes breast tissue and reshapes the chest to create a flatter, more traditionally masculine appearance. This surgery is often a foundational step in a multi-stage transition journey for many transgender men and non-binary individuals.

This procedure can provide significant emotional relief for individuals seeking alignment between their chest and gender identity. Top surgery isn’t just about changing your body—it’s about reclaiming your reflection and living more freely in the world.

What to Expect

During the Procedure
Top surgery is tailored to your body and goals. Our surgeons offer several techniques, including:

  • Double-incision mastectomy with nipple grafts for larger chests
  • Peri-areolar or keyhole incisions for those with smaller chests
  • Nerve-sparing methods to help preserve sensation
  • Hidden-scar approaches for a more natural-looking result

Surgery is performed under general anesthesia and typically takes 1–3 hours. Most patients go home the same day or after a one-night stay.

Risks and Recovery
Recovery after top surgery typically lasts two to six weeks, depending on the surgical approach and how your body heals. During this period, you'll wear a compression vest for about four weeks to support healing and minimize swelling. It's normal to experience some soreness and swelling in the initial days, which can be effectively managed with medication provided by your care team.

Most patients can return to work, school, or daily activities within two to three weeks, though it's important to avoid strenuous movement until your surgeon clears you. Scar care and sensation tracking continue over the next six months, with your surgical team monitoring healing and providing resources for optimal results.
